What about electronic submission of documents?

If you have already made your living will or DNAR and have access to a scanner, you can send your document(s) electronically (PDF files only). We will acknowledge receipt and send your membership card. You must ensure your document(s) are legible and valid.

1. Register as a user of the Respect Our Wishes website.

To register as a user of this website you must create a login. Some personal details will be required so your documents can be created and personalised. 

2. Draft your living will or do not attempt resuscitation directive.

A series of questions will guide you through the process of creating a Living Will or advance directive. As you draft your Living Will all your answers will be temporarily stored. This ensures that in the event you are interrupted you can pick up again where you left off. A progress bar will show you how much of the questionnaire you have completed.

3. Print and read through your document(s)

We suggest you print your document(s) as many people find it easier to read a paper copy. Read through your document(s) to ensure you are happy with the health care choices you have made.

4. Sign the document(s) making sure someone witnesses your signature.

For your document(s) to be valid they must be signed, dated and witnessed. If they are not we will return them to you and they will not be placed on the National Register of Living Wills.

5. Post the document(s) to Respect Our Wishes

You must post all the documents you have created that you want storing on the National Register of Living Wills to us. Our address is

6. We will scan your document(s)

The whole of the original document will then be available for view on the National Register of Living Wills by you and your medical team when it is needed.

7. We will return your document(s) to you by post with your membership card.

This may take up to three weeks.
You should keep the original document somewhere safe. We recommend you carry your membership card at all times in case of a medical emergency. To view your Living Will or Advance Directive the medical team must know your membership number (which will be on the card).

8. Enjoy the full range of benefits membership of the national register of living wills.

If you do not renew your subscription then we must assume you no longer want your document(s) to be on the National Register of Living Wills and they will be removed from it.
