End of life care is improving but report reveals plenty of scope for better services.

Primary Care needs to identify who will die soon and then communicate better to improve end of life care services. Only 27% of  4487 deaths reviewed were anticipated by Primary Care and the patient placed on palliative care registers. Of those 27%, just over half were offered the opportunity to dscuss their end of life care preferences.

When people had discussed where they would like to die, and it had been properly recorded, they were more likely to die where they had specified. Recommendations of good practice are highlighted in End of Life Care in Primary Care - a national snapshot from Omega.
